How to make an application to the MHARB?

The application to the Mental Health Act Review Board may be made by a patient under the Mental Health Act or by someone on behalf of that person. The application form is available:

  1. Online;
  2. From the nurse, social worker or doctor at the hospital, designated facility or health facility; or
  3. By contacting the MHARB office directly

If you require assistance with completing the application form you may ask the nurse or doctor at the health facility for assistance, a friend or family member, a lawyer or contact the Mental Health Act Review Board.

Applicants to the Review Board have a right to legal counsel.  If you would like to speak with a lawyer free of charge, please contact:

Once you have completed the application you can give it to the nurse responsible for the unit at the health facility, or send it to the Mental Health Act Review Board by:
